Self Reflection Questionnaire

The below questionnaire is used by many Life Coaches to help clients dig deep within to gain insight on what is holding them back. Feel free to use this as often as you need so that you have an understanding of what your next steps need to be and why. Be sure to clear your mind by taking a hot shower, hot bath, ten cleansing breaths and or meditation prior to this self reflection.

What you will need:

A quiet and clean space to think clearly and not be interrupted for at least 30 minutes

Dim lighting, preferably a couple candles

Notebook or journal and pen

Once you have decided on the issue you would like to work on ask yourself the questions below and write your answer in your journal. It’s important that you actually answer the questions completely to create the necessary awareness. Don’t think too much about your answers. Trust your first thoughts and write the first thing that comes to mind.

  1. What is the issue you would like to work on?
  2. What effect does it have?
  3. What would you like to achieve?
  4. What would be really good about achieving it?
  5. How will you know you’ve achieved it?
  6. What specifically do you want to achieve from this self-coaching exercise during the next 30 minutes?
  7. What have you done so far in regards to addressing this issue?
  8. What has worked?
  9. What hasn’t worked?

10. What haven’t you been doing?

  1. Why haven’t you solved it yet?

12. Which parts are within your control?

13. What could you do?

14. What have you seen work?

15. What would you do if you were ten years younger?

16. If someone you knew and respected was in the exact same position regarding this issue, what might they do?

17. Think of someone you admire and respect, anyone at all that would have experience in dealing with this issue. Imagine you asked them for advice on this issue. What do you think they might say?

18. What would happen if you did nothing?

19. Of all the options so far which one interests you most?

20. What are you going to do?

21. What is the specific first action you will take and exactly when will that be?

22. Rate yourself out of ten, as to how confident you are that you will be doing this?

23. If your rating is less than 10, what will you do to increase it?

24. Rate, out of ten, how far you’ve moved forward on this issue since you started this exercise.

 Affirmations: The act or an instance of affirming; state of being affirmed, the assertion that something exists or is true. It is something that is affirmed; a statement or proposition that is declared to be true.
